Insurance

FSP Wellness is an "Out of Network" office with ALL insurance companies, and as such, it is our policy that payment is due at the time of service.

We accept cash, and all major credit cards, as well as HSA (Health Savings Account) / FSA (Flex Spending Account) cards (if your plan allows).

You MUST contact your insurance provider and check into your coverage for "Out of Network" chiropractic care prior to our visit;​

• Deductible (Out of pocket costs)

• Percentage of coverage after deductible has been met.

• Limitations on number of visits per year.

• Insurance is a contract between you and your carrier.

• We are happy to print a Super Bill that you may submit to your insurance for personal reimbursement.

Auto Accident

If you have medical payment coverage through your auto insurance company FSP Family Wellness Center will bill them directly.

​

If not we will bill your health insurance for additional care.

​

We will require you to sign a lien stating that all bills incurred at FSP Family Wellness Center will be paid when your case settles.

Sometimes there is a third party insurance company involved and we will work with you through all of the billing.
